シトクロムP450 2C19(Cytochrome P450 2C19)、CYP2C19は酵素である。このタンパク質は、混合機能オキシダーゼ系のシトクロムP450のひとつであり、多くのプロトンポンプ阻害薬、抗てんかん薬など、生体異物の代謝に関与する。ヒトにおいて、CYP2C19タンパク質はCYP2C19遺伝子によって符号化されている。[3][4] 21世紀初頭に臨床的に使用されている薬の、少なくとも10%に作用している肝臓の酵素で[5]、特に抗血小板薬のクロピドグレル、潰瘍の痛みの治療薬オメプラゾール、抗マラリア薬プログアニル、抗不安薬ジアゼパムといったものである[6]。
CYP2C19は、UniProt(英語版)にて、(R)-リモネン-6-モノオキシゲナーゼと(S)-リモネン-6-モノオキシゲナーゼとされている。
遺伝的多型と薬理
CYP2C19の発現についての遺伝的多型があり(主にCYP2C19*2、CYP2C19*3、CYP2C19*17)、コーカソイド(白人)の約3-5%、アジア系民族の約15-20%に、CYP2C19機能がなく代謝が弱い[7][8]。例えば、プロドラッグであるクロピドグレルでは、活性のためにこの酵素による代謝を必要とするが代謝されず、薬理学的効果に達しない可能性がある。CYP2C19の変異がある場合、ジアゼパム(セルシン)、オキサゾラム(セレナール)、オキサゼパム(英語版)、テマゼパムは避けるべきである[9]。(S)-mephenytoinや他のCYP2C19基質を代謝する、各人の能力に基づいて、広範代謝群と不完全代謝群に分類できる[8]。不完全代謝を予測する8種の対立遺伝子の変異が同定されている[8]。

CYP2C19阻害剤の力価(英語版)は、以下のように分類できる。
- 強 血中濃度の曲線下面積 (AUC)の5倍以上の増加、あるいは基質のクリアランスの80%以上の低下[10]。
- 中等度 AUCの2倍以上の増加、あるいはクリアランスの50-80%の低下[10]。
- 弱 AUCの2倍以下で1.25倍以上の増加、あるいはクリアランスの20-50%の減少[10]。
